This Website contains forms which allow users to provide personally identifiable information in order to submit feedback or inquiries to Quorum, sign up for our email list, or download information. When a user provides personally identifiable information (such as name, email address, and/or phone number), Quorum may use this information to respond to your inquiry or to provide additional information that we feel may be of interest to you.
We will not share your information without your permission unless required by federal, state, and local law, rule, and regulation. We will not sell or rent this information to anyone.
Additionally, the user may enter information into the restricted, secured areas of the Website (including the OnQ™ Portal), which we store in our secure servers.
Information Automatically Collected
We automatically receive and collect certain types of information whenever you visit the Website. For example, like many Websites, we collect non-personally identifiable user information, such as Internet Protocol (IP) addresses, date, time and navigation history of visitors to the Website. This information is used for system management and improving the content and navigation of the site. The Website also uses “cookies,” which are pieces of data that are stored on the user’s hard drive and which contain information about the user such as the time that the current visit occurred, whether you have been to the Website before and what site referred you to our Website. The cookies used by Quorum serve the purpose of authentication when users login into the secure portions of the Website. Additionally, cookies are used for Google Analytics.
Google Analytics is a web analysis service provided by Google and it helps Quorum understand how visitors engage with Quorum’s website. Quorum uses Google Analytics to process cookie and usage data. Google utilizes the data collected to track and examine the use of www.quorumreview.com, to prepare reports on its activities and share them with other Google services. Google may use the aggregated, de-identified data collected to contextualize and personalize the ads of its own advertising network. Quorum does not use Google Analytics to track, collect or upload any data that personally identifies an individual (such as a name, email address or billing information), or other data which can be reasonably linked to such information by Google.
Use of Information
We will use the information you submit to improve the Website and to better provide content on the Website that may be of interest to you. From time to time, Quorum may match personally identifiable information with non-personally identifiable information. In other words, we may link personal information (name, etc.) with visit information, such as pages visited or files downloaded. Requests for service, support or information may be forwarded as needed to best respond to a specific request. We may retain e-mails and other information sent to us for our internal administrative purposes and to help us serve customers better. Any information you submit to us is for the exclusive use of Quorum and its agents or contractors. It will not be shared with any other party unless required by federal, state, or local law, rule, or regulation or by order or request of a regulatory agency. We do not sell, trade or rent personal information about our users.
Quorum maintains security measures to protect against unauthorized access to individual and client information. These measures include physical and information technology security systems to help protect against the loss, misuse and alteration of information contained on or obtained through the Website. Quorum uses sophisticated encryption and authentication tools to protect the security of your personal information that you share with us. We use strict security measures to safeguard online transactions through the use of Secure Sockets Layer (SSL) with 128-bit encryption. Any connection between your web browser and an https-protected page on our site uses this level of encryption (including the Portal).
Additionally, Quorum restricts access to client information to its officers, employees, IRB Board members, and IRB consultants who need to know such information and who are required to protect confidential information. As internet security cannot be completely guaranteed, however, Internet transmission to or receipt of information from the Website by the user will be undertaken at the user’s own risk. Quorum provides no warranty as to the absolute security of information users transmit to or receive from the Website, but we will utilize reasonable best efforts to protect such information on our systems.

This Agreement shall be governed by Washington law excluding its conflicts of laws rules. The exclusive venue for any action between the parties shall be in King County, Washington and you hereby consent to such jurisdiction.
